If you or a covered dependent have a COVID-19-related visit with an in-network provider (office, urgent care, emergency department, Doctor on Demand telehealth visit) and a test is not available and the provider submits a confirmed COVID-19 diagnosis code in accordance with CDC guidelines, it will be covered at no cost to you.

If you or a covered dependent require inpatient and/or outpatient treatment for COVID-19, when treatment follows evidence-based COVID-19 treatment guidelines, it will be covered at no cost to you.
